Creation of a Performance Evaluation Form

In order to maintain high-quality operations, high-quality personnel play a major role in high-quality performance. To achieve and maintain the highest quality, personnel must have a clear picture of what the objectives are and what needs to be done to achieve those objectives. They need to have an understanding of how well they are performing the tasks as well. One of the ways you can provide feedback to your personnel and reduce stress and issues is to evaluate their performance.

For this assignment, create a 1–2 page personnel evaluation form with 10 basic types of common personnel matters that management rates for performance. Listed below are a few of the items that you can include in the personnel evaluation form.

  • Arrives to work on time.
  • Operates safely during workday.
  • Gets along with coworkers.

The personnel evaluation form should include the following information:

  • Fields for employee’s name, position title, employee number, etc.
  • Instructions on how the manager should perform the evaluation and complete the form.
  • A rating scale breakdown for each personnel matter used for evaluation (negative, positive, neutral, or needs improvement, meets expectations, exceeds expectations, or a similar rating) and an explanation of the rating scale.
  • Include an explanation of each personnel matter, so a review can be conducted by many supervisors. A numerical rating can also be used to define each item by quality.

You must answer two of the following questions.  The content of the questions may overlap somewhat, but your answers should not have much overlap with each other. Your completed exam must be submitted on Canvas by 11:59 PM ET on Sunday, February 28th.  Though length will vary depending on your style of writing, you must write at least 4 double-spaced pages for each question.  Be sure to address every aspect of the question.

Your answers must be submitted as a single document in Microsoft Word or pdf format.  The answers must adhere to the following guidelines:

1.    Describe the fundamental tenets of social disorganization theory, starting from its origins with Shaw and McKay and continuing through the more recent extensions such as the concept of collective efficacy by Sampson’s and others. Include a discussion of the empirical research that has tested the theory and its extensions.

2.    Compare and contrast the social disorganization perspective (e.g. Shaw & McKay and Sampson) with the differential organization perspective (e.g. Anderson’s “code of the street”).  How are the two perspectives compatible, and how are they incompatible?  Be sure to include a discussion of empirical research for each perspective. Given that they are considerably different in many ways, how do you think it is possible that empirical evidence has found support for both?

3.    Robert Sampson’s book, Great American City, makes the case that life is decisively shaped by where you live. Each chapter in the book supports this argument in a different way, including (but not limited to) thorough discussions of the theory of collective efficacy, the influence of disorder, and mobility experiments. Based on the arguments and research evidence in this book, describe two specific programs or policies that could be implemented by cities or neighborhoods to reduce levels of crime in neighborhoods.  For each of the two programs you propose, you must describe the program and explain why you would expect it to be successful in reducing crime based on the arguments made in the book. You cannot simply give your own opinion about what would reduce neighborhood crime levels. Each program must be supported by specific arguments made in the book.

  • Provide in-text citations for any information you draw from the readings (e.g. Recent research has found that…. (Kubrin and Wo 2016)).
  • Use a 12pt font such as Arial or Times New Roman, double-spaced
  • Use margins no larger than 1”.
  • At the beginning of each answer, clearly indicate which question number you are answering, but do not type the actual question in your document.
  • Grammar, spelling, punctuation, and formatting will be part of your grade.
  • A title sheet is not required.
  • Provide in-text citations for any information you draw from the readings (e.g. Recent research has found that…. (Kubrin and Wo 2016)).
  • You do not need a references/works cited page if all of your citations come from the course readings. If you draw on anything outside the course–which is absolutely not necessary–you should provide a separate references page at the very end of your submission. Any recognized format is fine (e.g. APA, MLA, Chicago style, etc). This reference page does not count toward your page count.
  • Avoid including direct quotes from the readings. Instead, put things into your own words and then provide a citation at the end of the sentence. You only need to include page numbers in your citation if you are citing a direct quote. The best submissions will have no direct quotes.

Allison dives into the Lovelace labs and starts asking some questions. After securing an interview with the Director, he assures you that there has been no recent Y. pestis work. “We do keep it here, but it’s kept on liquid nitrogen and hasn’t been accessed in some times.” Indeed, the logs don’t seem to indicate that any vials are missing. “Is there a way we can verify that they really aren’t to blame,” wonders your Director. You set off to think about that.

Alexandre gets into the mechanism for patient response. Proper precautions are now being taken for all of the patients and their close contacts. Fortunately there have not been other cases as secondary infections that have emerged. Every health district is on high alert though.

Kelly asks some questions about bubonic plague, which has been ruled out. That said, there are some interesting questions that were asked, one of them which asks “where did the patients go?” Emily proceeds with asking some more kinds of questions and has thought about a questionairre.

Your assistant helps put the information together. “Together, this should provide us with plenty of information,” says your Director. “Let’s take a look.” As you take a look at the overall results of the surveys, you see a commonality – all of the patients have recently traveled to Albuquerque over the previous weekend, January 15-17th, 2020. Questioning them on why that particular weekend, all of the patients indicate that they had attended the annual Albuquerque Comicon. There doesn’t seem to be any link to an animal exposure, or bites, etc. “The ramifications of this seem obvious. Are we the epicenter of this ourbreak?” asks your Director. “How could this have happened?”

Ed did some work looking at a map and thinking about flights. He showed that the airport is a hub between the coasts. Definitely some good work.

Ed also asks for MLVA analysis of the patients and to compare them with the lab. A panel is run and the outcome is shown.

From the plague patients from New Mexico, New Jersey, and New York, four strains emerged. Use the French website from class to analyze them.7 6 3 7 22 15 87 6 3 7 -1 16 87 6 4 7 13 15 97 6 3 7 -1 12 8

The MLVA from the liquid nitrogen tank at Lovelace on the Kirtland Airforce base has the following:8 8 8 9 5 7 6

“What can we do with this data?” asks the Director.

It looks like you have some more investigating ahead to do. What do you want to look into next?
